How a husband-and-wife team set out to change the way physical therapy is delivered in Alpharetta — and why it worked.
Advanced Manual Therapies was founded in 2018 by Dr. Grant Smith and Lauren Smith — a husband-and-wife team who shared a single, clear frustration: the healthcare system was getting in the way of actual care. Grant had earned his Doctorate in Physical Therapy from the University of Miami's Miller School of Medicine and gone on to complete a post-doctoral fellowship to become an Osteopractor through the American Academy of Manipulative Therapy. Lauren had graduated from Atlanta School of Massage in 2015 and spent years refining her expertise in neuromuscular therapy, deep tissue work, and medical massage. Together, they had the skills. What they wanted was a practice worthy of them.
The conventional clinic model didn't fit. Insurance-driven care means short sessions, rotating aides, and a relentless pressure to see more patients in less time. It meant providers couldn't think deeply about a case, spend real time with a person, or integrate treatments across disciplines. Grant and Lauren wanted to build something different — a practice where every appointment was one-on-one, every plan was built for the individual, and every session actually moved the needle. So they did.
They opened Advanced Manual Therapies in Alpharetta, Georgia with a straightforward model: cash-based care, direct access, no referrals required. HSA and FSA accepted. Patients book directly, work one-on-one with their provider for every minute of every session, and receive a treatment plan with real, defined goals — not an indefinite cycle of visits. The result is a practice that looks nothing like a traditional PT clinic and feels nothing like one either.
Since 2018, AMT has grown into a team of fellowship-trained physical therapists, pelvic health specialists, and licensed massage therapists who work in genuine collaboration.
